All 10 plans may not be offered in your state, yet all 10 are standardized and certified by the U.S. Department of Health and Human Services so that each plan provides exactly the same kind of coverage no matter what state you live in (except for Massachusetts, Minnesota, and Wisconsin, which have their own standardized plans).

Once you have completed the application you may return it by mail along with the first month's premium to:

All premiums must be made payable to Standard Life and Accident Insurance Company.

You can also choose to have your premium electronically deducted from your bank account by completing the Authorization for Electronic Funds Transfer Form. If you choose this pay method, you do not have to send a check with your application.

Your application can also be faxed to us at 1-888-550-3217. If you choose to fax the application to us and do not complete the EFT Form, you must mail your first month's premium (made payable to Standard Life and Accident Insurance Company) along with a copy of the application to Medicare Supplement Specialists.
