Medicare Part D

Medicare Part D is a prescription drug plan offered by private insurance companies.

Part D Premium for 2012:

  • Part D plans charge a premium that varies by state. If you belong to a Medicare Advantage Plan that includes Medicare prescription drug coverage, the monthly premium you pay may include an amount for prescription drug coverage.

CMS Part D Standard Benefits for 2012:(The Standard Benefit is the minimum allowable plan to be offered.)

  • Deductible amount is $320.
  • Initial Coverage Limit is $2,930.
  • Out-of-Pocket Threshold is $4,770.
  • Coverage Gap (donut hole) begins when you pay total drug costs (what you pay + what the plan pays) of $2,840 and ends when you reach $4,770 in total drug costs. In 2012, Part D enrollees will receive a 50% discount on the total cost of their brand name drugs while in the donut hole. The full retail cost of the drug will still apply to the catastrophic stage. Enrollees will receive a 14% discount on generic drugs while in the donut hole.
  • Catastrophic Coverage - The enrollee pays the greater of 5% or $2.60 for generic drugs and the greater of 5% or $6.50 for all other drugs in 2011.
  • Drug copays and coinsurance vary by plan.

Low Income Subsidy(LIS) or "Extra Help":

Full Low-Income Subsidy - Duals(people with Medicare and full Medicaid) who reside in long-term care facilities.

  • $0 copay for drugs on plan formulary

Full Low-Income Subsidy - Other people with Medicare and Medicaid, including those enrolled in a Medicare Savings Program (MSP).

  • Copay is $1.10 generic/$3.30 brand name, if income is less or equal to 100% FPL (Federal Poverty Level).
  • Copay is $2.60 generic/$6.50 brand name, if income is greater than 100% FPL or if MSP only. No copay after $6,657.50 limit.

Full Low-Income Subsidy - Non-duals with incomes less than 135% FPL.

  • Copay is $2.60 generic/$6.50 brand name. No copay after $6,657.50 limit.

Partial Low-Income Subsidy - Non-duals with incomes less than 135% FPL.

  • Co-insurance is 15% (up to $6,657.50). Copay is $2.50 generic/$6.30 brand name after $6,657.50 limit.
  • Annual Deductible is $65.

Partial Low-Income Subsidy - Non-duals with incomes between 135% and 150% FPL.

  • Co-insurance is 15% (up to $6,657.50). Copay is $2.50 generic/$6.30 brand name after $6,657.50 limit.
  • Annual Deductible is $65.
